Granny’s Helpful Hands, LLC  makes a measurable difference in the lives of families each day. We serve medically fragile children, teens, and adults. Our nurses provide extended-hours skilled care in clients’ private homes in and around the 7-county Twin Cities metro area.

With our unique staffing and scheduling method, each nurse usually works with just one case. This allows the  nurse and client to establish a high level of trust–and that leads to better care. Every nurse has the opportunity to meet the client as part of the placement process. When both client and nurse agree to the match, everyone feels more comfortable.

Essential Qualifications

  • Current MN Nursing License: Registered Nurse or Licensed Practical Nurse (RN or LPN)
  • Current CPR for Healthcare providers

Preferred Qualifications

  • Home Care Experience
  • Trach and Vent hands-on experience within the last 24 months
  • Experience ordering and organizing supplies

Essential Functions
Competent medical care for client, including timely assessments and appropriate interventions and therapies to maintain medical stability or identify areas of possible concern.
Appropriate interaction with client to support client’s social and emotional goals.

Additional Functions
Professional approach to working with other staff members.

Physical Requirements

  • Ability to read and speak English
  • Ability to hear
  • Ability to lift 25 lbs
  • Ability to manage transfers of client

Candidates must be able to prove eligibility to work in the United States within 3 days of hire.

Pay Ranges:
RN–$19-$26/hour; LPN–$15-$20/hour

 

Seeking Qualified Professional-LSW/RN:

We are looking for a QP for our pca program, we are seeking a Qualified Professional who must be an RN or LSW  in the metro area. Position would be a flexible position, you would make your own schedule. Responsibilities include conducting  home visits for clients, supervision of pca’s , conducting bi monthly meetings with the client and pca, completion of initial Plan of Cares for new clients, working directly with clients when an increase of services is needed and preparing the request, etc. If this is something that you are interested in, please submit an application along with your resume for consideration.

 Internship/Practicum Oppurtunities:

We are currently seeking graduate level students for practicum opportunities.  We are also seeking Mental Health Professionals for supervisor and outpatient/in-home therapist.
The outpatient/in-home therapist/intern must be graduate level or post graduate working toward liscensure.  They will provide culturally competent mental health services to individuals and families who are experiencing varying levels of difficulties due to life situations or mental health diagnoses.
The outpatient therapist must possess:
  1. Knowledge of family systems and their impact on mental health within the family.
  2. Skill in applying varied interventions with individuals/families in crisis.
  3. Skill in working with a variety of cultures.
  4. Skill in working independently and implementing time management strategies.
  5. Willingness to maintain a flexible schedule.
  6. Ability to provide necessary self-care.
  7. Awareness of personal strengths and appropriate timing for referrals.
The outpatient therapist is required to:
  1. Maintain good standing with the appropriate board and insurance entities.
  2. Complete all recommended continuing education requirements (30 hrs. every two years-LMFT, LICSW, LPCC-40 hrs. every two years-Psychology and LPCC).
  3.  Secure client information per HIPAA standards.
  4. Maintain case files that are up to date as described below:
 i. Provide a diagnostic assessment and intake interview.
 ii. Develop and implement a culturally appropriate treatment plan and crisis plan (as appropriate). Treatment plans must be reviewed every 90 days.
iii. Ensure that all necessary releases and forms are current.
iv. When closing a file include a closing letter and completed discharge summary.
Clinical Supervisor must possess a professional license in order to provide clinical supervision in their prospective fields.  They will provide culturally competent clinical supervision to staff who provide counseling services to individuals and families who are experiencing varying levels of difficulties due to life situations or mental health diagnoses.
The Clinical Supervisor must possess:
  1. Knowledge of family systems and their impact on mental health within the family.
  2. Skill in applying varied interventions with individuals/families in crisis.
  3. Skill in working with a variety of cultures.
  4. Skill in working independently and implementing time management strategies.
  5. Willingness to maintain a flexible schedule.
  6. Ability to provide necessary self-care.
  7. Awareness of personal strengths and appropriate timing for referrals.
  8. At least two years of supervisory experience
The Clinical Supervisor is required to:
  1. Maintain good standing with the appropriate boards and insurance entities.
  2. Must have completed required hours of supervision training based on appropriate board’s criteria.
  3. Complete all recommended continuing education requirements (30 hrs. every two years-LMFT, LICSW, LPCC-40 hrs. every two years-Psychology and LPCC).
  4. Secure client information per HIPAA standards.
  5. Meet with assigned staff weekly, individually and once monthly in a group setting to review, update and sign all documents (diagnostic assessments, treatment plans, progress notes).
  6. Report directly to Clinical Director.
